Acute Care

At Heritage Crossing we offer a continuum of subacute and post-acute care. Our rehabilitation and other services are delivered in community-based settings where individuals receive structured support and excellent care. Many of our programs integrate work and social experiences to help individuals build skills, develop self-esteem and actively participate in the community. Heritage Crossing provides several levels of acute care as described below:

COMPLEXITY – Level 1: Medically stable and able to tolerate therapeutic activity


Skilled Nursing, PT, OT, ST, RT, SW, RD
Basic Wound Care
Enteral Feeding PEG/GT supplies
Podiatry initial and Quarterly
Incontinent supplies and Care, Bowel and Bladder
Haircut
DME – non specialty
Case Management
Stable Trach – no suctioning
No recent changes in treatment such as major meds, surgery, intubation/extubation or diet changes
Rehab PT, OT, ST – 7 hours a week
Basic Resp Care, Trach Changes and Supplies
Dental Initial and Annual
Lab and Radiology at Facility
Laundry Services
Activities
Tub bathing
Manageable Pain
No Drains
Includes Non-Emergency Transport for medical reasons (otherwise approved by management)

COMPLEXITY – Level 2: Medically stable and able to tolerate therapeutic activity

 Complex Wound Care
 Vent Services or IV Meds
 Enteral Feeding PEG/GT supplies
 Podiatry initial and Quarterly
 Incontinent supplies and Care, Bowel and Bladder
 Haircut
 DME – non specialty
 Case Management
 Stable Trach – with continuous O2 moderate suctioning
 No recent changes in treatment such as major meds, surgery, intubation/extubation or diet changes
 Foley, rectal Tube, PEG/GT
 Rehab PT, OT, ST – 10 hours a week
 Basic Resp Care, Trach Changes and Supplies
 Dental Initial and Annual
 Lab and Radiology at Facility
 Laundry Services
 Activities
 Tub bathing
 Moderate Pain
 Drains
 Internment Pulse OX
 Includes Non-Emergency Transport for medical reasons (otherwise approved by management)
 Behaviors up 10hrs 1/1 sitting

COMPLEXITY – Level 3: Medically stable and able to tolerate therapeutic activity


 Skilled Nursing, PT, OT, ST, RT, SW, RD
 Complex Wound Care
 Wound Vac
 Enteral Feeding PEG/GT supplies
 Podiatry initial and Quarterly
 Incontinent supplies and Care, Bowel and Bladder
 Haircut
 DME – any specialty
 Case Management
 Stable Trach – with continuous O2, continuous suctioning
 No recent changes in treatment such as major meds, surgery, intubation/extubation or diet changes
 Foley, rectal Tube, PEG/GT
 Rehab PT, OT, ST – 12 hours a week
 Basic Resp Care, Trach Changes and Supplies
 Vent Services AND IV Meds
 Dental Initial and Annual
 Lab and Radiology at Facility
 Laundry Services
 Activities
 Tub bathing
 Severe Pain
 Drains
 Continuous Pulse OX
 Includes Non-Emergency Transport for medical reasons (otherwise approved by management)
 Behaviors up 24hrs 1/1 sitting
